If you listen more carefully in Steve Jobs keynote

First he said "MOST OF THE COUNTRIES" will be selling at US$199 not "ALL COUNTRIES"

Second he did not mention that the iPhone 3G should be selling with a phone contract and US$199 is not a pay-as -you-go price
